Rose City Nursing and Rehab is seeking an experienced Registered Nurse Assessment Coordinator (RN MDS Coordinator) to join our skilled nursing facility in Lancaster, PA.
In this vital role, you will coordinate and oversee the resident assessment process, ensuring timely and accurate completion in accordance with state regulations and facility policies. (This is not a remote position)
Shift Available:
Responsibilities:

Routinely assess residents according to scheduled timelines or as needed due to condition changes or hospital returns.

Accurately observe, assess, and communicate resident condition changes to appropriate personnel.

Ensure completion of assessments and CAA’s by interdisciplinary departments (Social Services, Activities, Dietary) and collect necessary signatures.

Complete MDS, quarterly reviews, CAA’s, and care plans as needed.

Initiate and update care plans based on resident needs identified through assessment protocols.

Prepare and distribute MDS schedules to the interdisciplinary team.

Transmit MDS data to the state weekly (or as required) and maintain accurate documentation, including transmittal logs.
